Critical error when using Give

New problem ..

When using the give plugin ,when changing templates , 2 templates are on top of each other – please see https://www.mentalhealthlottery.co.uk/donations/veteranmentalhealth-v2/

If you change the template you get a critical error and the site shuts down.

Hello sir,

I have investigated your problem. The plugin provides three styles to choose from. You have activated the Classic form, which results in some unwanted HTML DOM elements appearing in your form. If you want to remove these, you’ll need to modify the custom CSS. I have hidden the incorrect submit button using CSS; please use our provided submit button instead. If you wish to customize your own style, please do so using custom CSS. After form submission, you’ll be redirected to the success page. Please select the correct success page from the donation settings. I have created a video and attached some screenshots. Please review them.
